After 1.e4 e5 2.Nf3 the main alternative to 2...Nc6 is undoubtedly Petroff's Defence, 2...Nf6. Proving an advantage against this defence has taxed the brains of World Champions. White's two main tries are 3.Nxe5 and 3.d4. The former has the defect that after 3...d6 4.Nf3 Nxe4 5.d4 d5 6.Bd3, black has a number of systems (6...Nc6, 6...Bd6 and 6..Be7), none of which are obviously bad. Playing 3.d4 restricts Black's options more, but white must be prepared to enter some long theoretical variations if he hopes to gain the advantage; nowadays it's still the question if white can achieve any advantage from the opening at all.
